Before you book non-emergency medical transportation (NEMT), you should verify that the provider is licensed, insured, accredited, and equipped to transport you safely. The NEMT industry includes hundreds of operators across Illinois alone, and not all of them meet the standards required for safe medical transport. This guide walks you through six specific checks you can perform in under 15 minutes to confirm your provider's credentials.
NEMT is different from a standard car ride. Patients using NEMT often have mobility limitations, medical conditions that require monitoring during transport, or post-surgical restrictions that demand specific vehicle equipment and driver training. Booking with an unverified provider puts your health and safety at risk.

Taking 15 minutes to verify credentials before your first ride can prevent missed appointments, unsafe transport conditions, and billing problems. Here are the six steps.
The National Provider Identifier (NPI) is a unique 10-digit number assigned by the Centers for Medicare & Medicaid Services (CMS) to healthcare providers. NEMT companies that bill Medicaid, work with healthcare facilities, or operate as healthcare service providers should have an NPI number.
The NPI should be Type 2 (Organization), not Type 1 (Individual). A Type 2 NPI means the business entity itself is registered — not just an individual person. The taxonomy code should relate to transportation services. The address should match the provider's claimed business location.

If the provider cannot give you an NPI number, or the number does not appear in the NPPES registry, that provider is not registered with CMS. This does not necessarily mean they are illegitimate — some private-pay-only providers may not have an NPI — but it limits their ability to coordinate with healthcare facilities and bill insurance. For Medicaid-covered transport, an NPI is required.
The Better Business Bureau (BBB) evaluates businesses on trust, transparency, and complaint handling. BBB Accreditation is voluntary — a business must apply, pass a review, and agree to participate in dispute resolution.
An accredited business with an A or A+ rating and zero or few complaints is a strong indicator of reliability. Pay attention to how the company responds to complaints — BBB shows whether complaints were resolved, unanswered, or unresolved.

An F rating, multiple unresolved complaints, or “Not BBB Accredited” status combined with complaints should give you pause. While many legitimate small businesses are not BBB accredited, a pattern of unresolved complaints is a clear warning sign.
Google reviews provide real patient and family feedback that is difficult to fake at scale. Unlike testimonials on a company's own website, Google reviews are posted by verified Google users and include dates, ratings, and detailed comments.
Look for specific details in reviews: mentions of on-time pickups, driver professionalism, vehicle cleanliness, wheelchair handling, and communication. Generic one-line reviews (“Great service!”) are less informative than detailed accounts of actual transport experiences.

Providers with fewer than 5 reviews, a rating below 4.0, or multiple reviews mentioning missed pickups, rude drivers, or billing problems should be approached with caution. Also watch for providers with no Google Business Profile at all — this often means they are very new or operating informally.
NEMT providers must carry specific insurance policies that differ from standard personal auto insurance. The exact requirements vary by state, but in Illinois, NEMT providers generally need:
Ask the provider directly: “Can you provide a Certificate of Insurance (COI) showing your commercial auto, general liability, and professional liability coverage?” A legitimate provider will produce this document readily — healthcare facilities require COIs from NEMT providers as a condition of partnership.
For licensing, check with your state's Department of Transportation or Secretary of State business registry to confirm the company is legally registered and in good standing.
Any provider who refuses to show proof of insurance, claims they only carry personal auto insurance, or cannot produce a COI is not properly insured for NEMT. Do not book with them. In the event of an accident, personal auto insurance will deny the claim because the vehicle was being used for commercial purposes, leaving you with no coverage.
If you use a wheelchair, scooter, or need accessible vehicle features, verify that the provider's vehicles meet Americans with Disabilities Act (ADA) requirements.
Ask to see the vehicle before your first ride, or request photos showing the ramp/lift and securement system. A provider with proper ADA-compliant vehicles will be happy to show them — they invested significantly in this equipment.

If a provider claims to offer wheelchair transport but uses standard minivans with portable ramps and no floor-mounted securement system, the wheelchair is not properly secured during transit. This is both an ADA violation and a safety hazard.
NEMT providers handle protected health information (PHI) as part of every trip: your name, address, medical facility, appointment type, and sometimes diagnosis or treatment information. Providers who work with healthcare facilities and insurance companies are required to comply with the Health Insurance Portability and Accountability Act (HIPAA).
Your medical transportation records contain sensitive information. A provider without HIPAA compliance may store your data insecurely, share your information with unauthorized parties, or lack the protocols to protect your privacy. For patients using Medicaid-funded transport, HIPAA compliance is not optional — it is a federal requirement.
A provider who does not know what HIPAA is, has no written privacy policy, or cannot explain how they protect patient data is not handling your information responsibly.
