What information do you need to schedule a ride?
We need the patient’s name, pickup address, destination address, appointment date and time, mobility level (ambulatory, wheelchair, or stretcher), and any special equipment needs like oxygen or a Broda chair. Insurance status is helpful but not required to book a ride.
Can I book rides on behalf of my patient?
Yes. Social workers, discharge planners, case managers, family members, and facility staff can all book rides on behalf of patients. Just call (708) 505-6994 and let our dispatch team know you are booking for someone else.
