Dream Care Rides accepts Medicaid trips originating within 6.5 miles of our Olympia Fields base (20000 Governors Dr, Olympia Fields, IL 60461) or our Palatine base. The form above checks your pickup address automatically. If you fall outside our service area, call (708) 505-6994 and we can discuss options.

Medicaid transportation requires Prior Authorization (PA) from your managed care organization. The PA process takes 3–5 business days and is designed for standing orders — patients who need 2 or more trips per week to the same facility. One-time rides are better handled through your MCO's transportation broker. For recurring dialysis, chemo, therapy, or specialist visits, this form is the right starting point.

If your pickup address is outside the 6.5-mile radius, you have two options: (1) contact your MCO's transportation broker for a provider closer to you, or (2) call Dream Care Rides at (708) 505-6994 to ask about private pay rates for your route. Private pay ambulatory rides start at $35–$65 base plus $2–$4 per mile.

Through Medicaid, Dream Care Rides provides ambulatory (sedan/SUV) and wheelchair (ADA van) transportation. Both include curb-to-curb and door-to-door service levels. Once your Prior Authorization is approved by your MCO (typically 3–5 business days), Dream Care Rides can begin service within 24–48 hours.
