Bariatric medical transportation is a category of non-emergency medical transportation (NEMT) designed for patients whose body weight exceeds the safe operating limits of standard transport vehicles. Most standard NEMT wheelchair vans, lifts, ramps, and securement systems are rated for passengers up to 300 to 350 pounds. For patients who weigh more, these standard components may be structurally unsafe, physically uncomfortable, or entirely unusable. Bariatric NEMT addresses this gap with purpose-built vehicles, reinforced equipment, and crews trained in safe bariatric patient handling.
Patients who typically need bariatric transportation include individuals weighing over 300 pounds who require wheelchair van transport, patients weighing over 400 pounds who need stretcher transport, individuals preparing for or recovering from bariatric surgery (gastric bypass, sleeve gastrectomy, or lap-band procedures), bariatric patients receiving recurring treatments such as dialysis or chemotherapy, patients being discharged from hospitals or skilled nursing facilities, and individuals transferring between care facilities who cannot safely ride in a standard ambulette.
Dream Care Rides provides bariatric non-emergency medical transportation throughout Chicago, Cook County, the south suburbs, the western suburbs, and Northwest Indiana. We operate our own bariatric fleet with reinforced wheelchair vans, bariatric stretchers rated to 700 pounds, heavy-duty hydraulic lifts rated to 800 pounds, and trained two-person crews on every bariatric trip. Call (708) 505-6994 to book.
Bariatric transportation is not limited to a single medical condition. Any patient whose weight exceeds the safe capacity of standard NEMT equipment may need bariatric transport. Common scenarios include:
Dream Care Rides operates a dedicated bariatric fleet that is purpose-built for larger patients — not standard vehicles retrofitted with aftermarket parts. Every component of our bariatric vehicles is rated for the weight it will carry. This distinction matters because a standard wheelchair van with an aftermarket lift bolted on may technically raise a heavier patient, but the floor tracks, securement hardware, suspension, and chassis were not engineered for that load. Our bariatric fleet eliminates that risk entirely.
We serve bariatric patients across three weight tiers, each with appropriate equipment and vehicle configurations:
TIER 1
300 – 400 lbs
Reinforced wheelchair van with wide-body wheelchair, heavy-duty ramp, and reinforced securement. Single driver may be sufficient for seated patients.
TIER 2
400 – 500 lbs
Bariatric wheelchair van or stretcher vehicle with hydraulic lift, two-person crew, and reinforced floor tracks. Pre-transport site assessment required.
TIER 3
500+ lbs
Bariatric stretcher vehicle with heavy-duty hydraulic lift (800 lbs rated), two-person crew, and custom equipment configuration. Call (708) 505-6994 to discuss.
| Specification | Bariatric Wheelchair | Bariatric Stretcher |
|---|---|---|
| Weight Capacity | Up to 400 lbs (seated) | Up to 700 lbs (supine) |
| Lift Capacity | 800 lbs hydraulic | 800 lbs hydraulic |
| Door Width | 36 inches minimum | 36 inches minimum |
| Interior Width | 60 inches clear | 60 inches clear |
| Stretcher Features | N/A | Adjustable head/knee, full side rails, padded surface |
| Crew | 1–2 person crew | 2-person crew (always) |
| Oxygen Support | Available (+$25) | Available (+$25) |
Every bariatric vehicle is climate-controlled, cleaned and sanitized between trips, and equipped with the hardware necessary to secure bariatric wheelchairs and stretchers during transit. We inspect and maintain our bariatric fleet on an accelerated schedule to ensure that every component meets or exceeds manufacturer specifications. If your weight exceeds 400 pounds for seated transport or 700 pounds for stretcher transport, call us at (708) 505-6994. We never turn a patient away without exploring every option.
Bariatric transportation requires specialized equipment that costs more to purchase, maintain, and operate than standard NEMT equipment. A bariatric surcharge of $30 to $100+ is applied on top of the base rate for the transport type (wheelchair or stretcher). The surcharge amount depends on the specific equipment configuration required for the patient.
| Transport Type | Base Rate | Per Mile | Bariatric Surcharge |
|---|---|---|---|
| Ambulatory (sedan/SUV) | $35 – $65 | $2 – $4/mi | N/A |
| Bariatric Wheelchair | $65 – $115 | $3 – $6/mi | $30 – $60 |
| Bariatric Stretcher | $300 – $525 | $5 – $16/mi | $50 – $100+ |
Additional surcharges may apply: weekends 1.5×, holidays 2.25×, wait time $15–$30 per 15 minutes, supplemental oxygen $25, and stairchair $25. All pricing is confirmed upfront before the ride is dispatched. There are no hidden fees. Book online or call (708) 505-6994 for your exact quote. Use our NEMT cost calculator for an estimate.
Understanding typical use cases helps families and discharge planners determine whether bariatric transport is the right choice. Here are the most common scenarios we serve in Chicago:
Patients preparing for bariatric surgery (gastric bypass, sleeve gastrectomy, duodenal switch, or lap-band) attend multiple pre-operative appointments: surgeon consultations, cardiac clearance, pulmonary function tests, blood work, nutrition counseling, and psychiatric evaluations. These appointments often occur at different facilities over several weeks. After surgery, patients need transport home from the hospital and to follow-up visits with the surgeon, nutritionist, and primary care physician. Dream Care Rides provides bariatric wheelchair transport for ambulatory patients and bariatric stretcher transport for patients who cannot sit upright after surgery.
Hemodialysis patients travel to treatment three times per week — 156 trips per year. For bariatric patients, standard wheelchair vans may not accommodate their weight safely, and the post-dialysis fatigue that most patients experience makes stretcher transport medically appropriate for some. Dream Care Rides sets up standing orders for bariatric dialysis transport so rides are dispatched automatically without requiring the patient to call before every session.
Hospital discharge planners frequently contact Dream Care Rides when a bariatric patient is ready for discharge but the facility's standard transport options cannot safely accommodate the patient's weight. We coordinate with the discharge team, arrive with the correct bariatric vehicle and two-person crew, and transport the patient home or to the receiving facility. For hospital discharge transportation, we can often accommodate same-day requests for private pay patients.
Bariatric patients in skilled nursing facilities or rehabilitation centers need transport to external medical appointments (imaging, specialist consultations, outpatient procedures) and may eventually need transport for discharge. Our pre-transport site assessment ensures we know the facility's layout — elevator dimensions, hallway widths, room configurations — before the first trip so there are no surprises on transport day.
Safe bariatric transportation requires more than a heavy-duty vehicle. It requires a systematic approach to every aspect of the transport — from the initial site assessment through the final patient transfer at the destination. Dream Care Rides follows a standardized bariatric safety protocol on every trip.
Before the first trip, our team surveys the pickup and drop-off locations. We measure doorway widths, hallway clearances, elevator dimensions, and identify stairs or obstacles. This ensures we bring the right equipment and crew configuration so there are no delays or improvisation on transport day.
Every bariatric stretcher trip and most bariatric wheelchair trips are staffed by a two-person crew. This is not optional or an upgrade — it is our standard operating procedure. Two trained crew members ensure safe transfers, proper body mechanics, and patient stability throughout the entire transport.
Before departure, the crew verifies that all equipment is rated for the patient's weight: wheelchair or stretcher, lift or ramp, securement hardware, and safety harness. No component is used beyond its rated capacity. Equipment is inspected before every trip and maintained on an accelerated service schedule.
Our crews complete specialized sensitivity training focused on bariatric patient care. They communicate clearly and respectfully, ask about comfort preferences, use appropriately sized equipment, and treat every interaction as routine — because for our bariatric fleet, it is routine. We never make a patient feel like an accommodation.
Many bariatric patients have had negative experiences with transportation — narrow doorways that require squeezing through, lifts that strain under weight they were not designed to carry, or drivers who visibly struggle with equipment. Every one of those moments erodes a patient's willingness to attend future medical appointments. Dream Care Rides eliminates those moments with purpose-built equipment, properly trained crews, and a transport experience designed from the ground up for bariatric patients.
Dream Care Rides provides bariatric wheelchair and bariatric stretcher transportation throughout Chicago, the suburbs, and Northwest Indiana. Our headquarters is at 20000 Governors Dr Suite 103H, Olympia Fields, IL 60461. We serve all major hospitals, dialysis centers, skilled nursing facilities, and rehabilitation centers in the Chicago metro area.
For long-distance bariatric transport beyond the Chicago metro area — including interstate transfers to Indiana, Wisconsin, or Michigan — see our national bariatric transportation page or call (708) 505-6994.
We recommend booking bariatric rides at least 72 hours in advance to ensure the correct vehicle and crew are available. For recurring appointments, we set up standing orders so your rides are dispatched automatically.

Many insurance plans cover bariatric non-emergency medical transportation when a physician certifies the medical necessity. Illinois Medicaid managed-care plans frequently cover both bariatric wheelchair and bariatric stretcher transport. Medicare Advantage plans may cover bariatric NEMT depending on the specific plan benefits. A Physician Certification Statement (PCS) is typically required for stretcher transport regardless of payer.
Dream Care Rides' billing team verifies your insurance benefits, obtains the PCS from the treating physician if needed, coordinates with Medicaid brokers for prior authorization, and handles all paperwork so you can focus on the medical appointment rather than the logistics. For Medicaid bariatric rides, contact us at least 5 business days before your appointment so we can complete the authorization process.
For patients who prefer private pay, no prior authorization or PCS is required for wheelchair transport. You receive an upfront price quote and pay at the time of service. Private pay patients receive priority scheduling for same-day and next-day requests.
