Apply to Our Network

Fill out the application below. We verify your vehicle standards, driver qualifications, insurance, and licensing. Approval takes 5-7 business days.

Complete Onboarding

We train you on our dispatch platform, Medicaid documentation requirements, trip logging, and member interaction standards. This ensures clean billing from day one.

Start Receiving Trips

We assign recurring Medicaid rides based on your location, vehicle type, and availability. Trips come through our platform with PA numbers already attached.

Drive & Document

Complete the trip, log pickup/dropoff times, capture member signature, and submit through the platform. Clean documentation means fast payment.

Get Paid

We submit claims to Medicaid on your behalf and remit payment to you after reimbursement. Transparent reporting shows every claim, its status, and your earnings.

Illinois Medicaid NEMT Rates

Billing Codes & Reimbursement

Illinois Medicaid NEMT is billed using these HCPCS codes. Rates are county-specific and set by the Illinois HFS fee schedule. Illinois increased NEMT rates by approximately 40% in FY 2024.

T2003

Ambulatory (Service Car / Medicar)

Walk-on patients who can sit in a standard sedan or SUV. Most common Medicaid NEMT ride type.

Mileage: A0130 (per loaded mile)

T2003 + modifier

Wheelchair Van

Wheelchair-bound patients requiring ADA-compliant van with hydraulic lift. Higher reimbursement than ambulatory.

Mileage: A0130 (per loaded mile)

T2005

Stretcher Van

Patients requiring supine transport. Highest NEMT reimbursement. Requires IDPH stretcher van licensing.

Current fee schedules are published by Illinois HFS at hfs.illinois.gov/medicalproviders/medicaidreimbursement/transportation. Rates vary by Cook County, Will County, and other counties.

Vehicle Requirements

  • PT or MC license plates (Illinois)
  • Current vehicle registration
  • Vehicle inspection passed within 12 months
  • ADA-compliant wheelchair lift (for wheelchair vehicles)
  • Clean, well-maintained interior
  • $250,000 bodily injury / $50,000 property damage insurance minimum

Driver Requirements

  • Valid Illinois driver's license (CDL not required for most NEMT)
  • Clean driving record (no DUI, reckless driving, or serious violations)
  • Criminal background check clearance
  • Safety training certification (per Public Act 95-501, every 3 years)
  • CPR / First Aid certification (preferred)
  • Professional appearance and demeanor

Business Requirements

  • Active business entity (LLC, Corp, or Sole Proprietor)
  • W-9 on file
  • General liability insurance
  • Workers compensation (if applicable)
  • Willingness to use our dispatch platform
  • Commitment to on-time performance (95%+ target)
