Short answer: It depends on the service level. Routine ambulatory and wheelchair NEMT in Illinois usually does not need separate prior authorization beyond the broker's standard trip authorization. Stretcher transport and certain long-distance trips do require PA with a Certificate of Medical Necessity (CMN).
For ambulatory (sedan) and wheelchair-van NEMT trips to routine covered appointments — primary care, specialist visits, dialysis, therapy — the broker's booking authorization is typically all that is needed. See our Medicaid NEMT coverage guide for the full picture.
Prior authorization with a CMN is generally needed for stretcher / gurney transport, long-distance interfacility transport, and some out-of-state trips. The full workflow is in our Illinois PA guide and the stretcher PA guide.

Usually the ordering provider's office or the NEMT provider submits the PA, depending on the MCO. Dream Care Rides handles PA submission for stretcher trips when needed.
Routine PAs are typically processed in 24-72 hours. Urgent or expedited requests can be faster. Always allow extra lead time when stretcher transport is involved.
A physician certification of medical necessity (CMN) is the core document. Diagnosis codes, level-of-service justification, and pickup/drop-off details are typically required.
