For Medicaid NEMT, you qualify if you have active Illinois Medicaid and no other way to get to your medical appointment. For Pace ADA paratransit, you must apply and demonstrate a disability that prevents use of fixed-route transit. (3) For Medicaid, call your MCO and request Dream Care Rides as your provider. We recommend booking 24-48 hours in advance for guaranteed availability.

For private-pay rides, no documents are needed — just your name, pickup address, and destination. For Medicaid rides, have your Medicaid ID number and appointment details ready.
