Rehabilitation Center NEMT Partnership

Rehabilitation patients are in active recovery. Their mobility changes week to week, sometimes day to day. A patient recovering from hip replacement surgery may need stretcher transport in week one, wheelchair transport in week three, and ambulatory transport by week six. A standard NEMT booking system that locks in a service level at the time of scheduling cannot accommodate this progression.
Transport reliability directly impacts therapy outcomes. Rehabilitation programs are designed with specific frequency and intensity targets — physical therapy three to five times per week, cardiac rehab three times per week, occupational therapy two to three times per week. Missed sessions disrupt the treatment protocol and can extend recovery timelines. When transport is unreliable, patients miss sessions, progress stalls, and the facility's therapy outcomes metrics suffer.
Driver familiarity also matters. A driver who has transported a rehab patient multiple times understands their current mobility level, preferred assistance methods, and any precautions. They know whether the patient can manage vehicle steps or needs the wheelchair lift. They know how much time the patient needs to get settled. This familiarity reduces fall risk during transfers and improves the overall transport experience.

Standing orders for rehab patients work the same way as our dialysis standing order system, with additional flexibility for the rehabilitation context. Your center provides the patient panel — names, addresses, therapy schedules, and current service levels. We program each patient into our dispatch system.
Transport dispatches automatically on each scheduled therapy day. When a patient's service level needs to change — from stretcher to wheelchair, or wheelchair to ambulatory — a single call to dispatch updates the standing order. The change takes effect on the next scheduled pickup.
We assign a primary driver to each recurring rehab patient when scheduling permits. The same driver handles the majority of that patient's transports, building familiarity with the patient's mobility status and preferences. When the primary driver is unavailable, we assign a backup from the same facility-trained team.
We provide non-emergency hospital-to-rehabilitation-facility transfers for patients transitioning from acute care. These transfers typically involve stretcher or wheelchair service, depending on the patient's condition at discharge.
As rehabilitation proceeds, we track the patient's service level progression. A patient who arrives at your facility via stretcher from the hospital may progress to wheelchair transport within the first weeks of therapy, and potentially to ambulatory transport before discharge from the rehab program. Each progression is a simple standing order update.
For patients discharged from your rehab facility to home, we provide the final transport and can continue with outpatient therapy transport if the patient's rehab program extends into the outpatient phase.
